DISCIPLINARY ACTION REPORT

 

Rebecca A. Parkhurst

On February 19, 2010, at a regularly scheduled meeting, the Minnesota Board of Marriage and Family Therapy approved a Stipulation and Order on

Rebecca A. Parkhurst of Deephaven, Minnesota, for violations of statutes and rules enforced by the Board.  The order causes Ms. Parkhurst to surrender her license to practice marriage and family therapy in Minnesota.  The stipulation is a public document and may be obtained from the Board office.

 

Heather A. Whatley

On August 18, 2006, at a regularly scheduled meeting, the Minnesota Board of Marriage and Family Therapy approved a Stipulation and Order on

Heather A. Whatley of St. Paul, Minnesota, for violations of statutes and rules enforced by the Board.  The order causes Ms. Whatley to surrender her license to practice marriage and family therapy in Minnesota.  The stipulation is a public document and may be obtained from the Board office.

 

Gary Graham

On December 17, 2004, at a regularly scheduled meeting, the Minnesota Board of Marriage and Family Therapy suspended the license of Gary Graham of

St. Elmo, Illinois, for failure to comply with a Stipulation and Order which he previously signed with the Board.  The suspension revokes Mr. Graham’s right to practice marriage and family therapy in Minnesota.

 

Barbara L. Steiner

On April 16, 2004, at a regularly scheduled meeting, the Minnesota Board of Marriage and Family Therapy approved a Stipulation and Order on Barbara L. Steiner, of Mora, Minnesota, for violations of statutes and rules enforced by the Board.  The order causes Ms. Steiner to surrender her license to practice marriage and family therapy in Minnesota.  It also prevents practice as an unlicensed mental health practitioner.  This Stipulation and Order is a public document and may be obtained from the Board office.

 

Beth M. Erickson

On September 19, 2003, at a regularly scheduled meeting, the Minnesota Board of Marriage and Family Therapy approved a Stipulation and Order on Beth M. Erickson of Edina, MN, for violations of statutes and rules enforced by the Board.  The order causes Ms. Erickson to surrender her license to practice marriage and family therapy in Minnesota.  It also prevents practice as an unlicensed mental health practitioner.  This Stipulation and Order is a public document and may be obtained from the Board office.

 

Randall S. Voeks

On December 21, 2001, at a regularly scheduled meeting, the Minnesota Board of Marriage and Family Therapy approved a Stipulation and Order on Randall S. Voeks for violations of statutes and rules enforced by the Board.  The order causes Mr. Voeks to surrender his license to practice marriage and family therapy in Minnesota.  It also prevents practice as an unlicensed mental health practitioner.  This Stipulation and Order is a public document and may be obtained from the Board office.

 

Roopnarine John Singh

On April 20, 2001, at a regularly scheduled meeting, the Board approved a Stipulation and Order on Roopnarine John Singh, for violation of statutes and rules enforced by the Board. This order causes Mr. Singh to surrender his license to practice marriage and family therapy in Minnesota. On May 19, 2000, at a regularly scheduled meeting, the Board approved a Stipulation and Order on Steven Alan Carlson of Burnsville, Minnesota, for violation of statutes and rules enforced by the Board.
